What We Do
Day Habilitation Program
Our qualified DayHab staff provided person centered programs that focus on formulating a positive vision of the future for the person, based on his or her capacities, strengths and preferences. We provide transportation services for pick-ups and drop-offs to our programs in week days (Monday through Friday).
Family Education and Training (FET)
Family Education and Training is provided to families of consumers who are enrolled in the Home and Community Based Waiver Service and who are under the age of 18. The purpose of FET is to enhance the decision making capacity of the family unit, providing orientation regarding the nature and impact of disabilities and traumatic brain injury upon the consumer and his/her family, and to inform them about other service alternatives.
Medicaid Service Coordination
Medicaid Service Coordination has increasingly become a critical element in helping consumers identify and access services so they can become more active and productive members of their communities. Working with the consumers and family members, our service coordinators advocate for services on their behalf. The primary objective is to enable consumers to participate in the community in a manner that is consistent with their personal goals, interest, and individual needs.
We also provide Community Habilitation. This service is provided both at home and in the community by trained staff.
We provide Residential and Hourly Respite Care to individuals in their homes. This service provides weekend relief for both the consumer and the primary caregivers or family members.
